Pathways to Community is a collaborative partnership between the Coconino County Sheriff’s Office, Coconino County Health and Human Services (CCHHS), and many other partner agencies, providing re-entry support services for individuals leaving the Coconino County Jail. Individuals being released from custody receive immediate connection to social, behavioral, housing, transportation, and legal services. Phase I included a 3,150 SF renovation to an existing structure and Phase II was a 1,506 SF addition.
